The Lipogems® system is a sterile single-use class II A disposable medical device intended for the closed-loop processing and transferring of autologous adipose tissue in a single surgical step.

The processing takes place in a closed- loop system in a single surgical step. The harvested tissue is minimally manipulated and used to repair, restore or replace damaged tissue.

This phase can be performed even under local anaesthetic. Following an infiltration of a solution composed of saline, adrenaline and lidocaine the harvesting can begin. Both procedures are performed using a series of blunt cannula that are connected to the Luerlock® syringe included in the kit.

Immediate Processing

The harvested tissue is processed immediately in the surgical area using the Lipogems® system. This phase permits a gradual reduction in the adipose tissue clusters and removes impurities (blood, oil and cell debris).

The Adipose Niche

Adipose tissue contains a specialized, natural microenvironmental niche that contains adipocytes, pre-adipocytes, pericytes, mesenchymal stem cells, microvasculature and other cells incorporated into a collagen matrix. Retention of this microarchitecture is key in sustaining the normal function of the niche during tissue healing and repair.

The micro-fragmented adipose tissue obtained with the Lipogems® device retains the structural properties and microarchitecture of the original tissue: the scaffold (the adipose tissue and the stromal structure), the cells (endothelium, pericytes/MSCs), and the growth factors (cytokines and chemokines).

Why Adipose Tissue

Scientifically, what we commonly call fat is called adipose tissue. It is a versatile tissue that plays an essential role for a healthy functioning body, in particular it has a notable role in healing processes.

For decades, numerous studies have been conducted on the healing potential of adipose tissue, as it contains an important network of repairing cells, which can be activated. All these special cells work together as a functional unit.

Adipose tissue is a complex tissue composed of many cell types including adipocytes, reticular cells, perivascular cells and mesenchymal cells, all immersed in a microenvironment called a niche. It also plays an important role as an energy reserve, in thermal insulation, in defense against trauma and in hormonal regulation.

The Damage Response Mechanism

Today it is widely recognized that in response to local tissue injury or inflammation, the body responds in a complex set of ways aimed at controlling the micro-environment. Because adipose tissue provides not only cushioning and support of so many normal tissues in the body, but also plays a role in the biochemical homeostasis of these tissues, it is only natural to expect the involvement of adipose tissue in the normal healing response setting. To this end, retention of the cellular and tissue microarchitecture of adipose tissue is important in maximizing the impact during normal healing processes.

Lipogems®

Lipogems® is a single-use device for the closed-loop processing of lipoaspirated adipose tissue intended for autologous use. The entire procedure takes place in a single surgical episode. Through minimal manipulation, without the use of enzymes or other additives, in a closed and aseptic system, there is a progressive reduction in the volume of adipose tissue clusters and the elimination of oil and blood residues. The entire procedure takes place in complete immersion in physiological solution, thus minimizing any traumatic action on the cell products. The product thus obtained is micro-fragmented, non-expanded adipose tissue intended for autologous tissue transfer.

The absence of gross physical destruction of tissue components (centrifuge), or enzymatic treatment, preserves tissue integrity and renders the tissue “minimally manipulated”. After processing with the Lipogems® device, the adipose niche, i.e. the main structural and morphological adipose unit, is kept intact and vital, and protects the cellular microarchitecture. Furthermore, the reduction in the size of adipose tissue clusters improves handling and ease of transplantation.
